Qualifications

• Associate's degree in Nursing required; Bachelor's degree preferred.
• RN license required.
• 3–5 years related work experience in acute care or managed care with two years in case management.
• Knowledge of discharge planning and utilization review practices including Interqual criteria.
• Experience with computer systems including web-based applications and Microsoft Office.

Responsibilities

• Perform utilization management, care coordination, and discharge planning for assigned caseload.
• Coordinate and review medical records for severity of illness and intensity of service using Interqual criteria.
• Create case management care plans using assessment, planning, implementation, and evaluation processes.
• Facilitate multidisciplinary collaboration daily.
