Job summary

You will manage education, discharge planning, and collaboration with multidisciplinary teams to optimize outcomes and ensure patient-centered care. Strong clinical judgment, evidence-based practice, and effective communication are essential.

Qualifications

• Current, full, active, and unrestricted RN license in a U.S. state or territory or D.C.
• English language proficiency for VA direct patient care per 38 U.S.C. 7403(f) and VA standards.
• Graduation from an approved nursing program; accreditation by ACEN or CCNE; licensure/certification per VA Qualification Standards.

Responsibilities

• Provide competent, evidence-based nursing care in inpatient and outpatient settings.
• Coordinate care, discharge planning, and education for Veterans and caregivers.
• Participate in interdisciplinary care management to meet complex health needs and optimize outcomes.
• Lead education, orientation, competency development, and quality improvement activities.

Education

Associate Degree in Nursing (ADN) or Diploma in Nursing

Bachelor of Science in Nursing (BSN)

Master's degree in nursing (MSN)
