Job summary

Sangre is seeking an RN Case Manager in Canon City, CO. The role involves coordinating patient-centered care plans and delivering compassionate home health services Monday–Friday, 8:00 AM–5:00 PM.

Expect meaningful patient and family interactions and teamwork in a supportive environment. The position offers competitive pay starting at $35/hr, mileage reimbursement, On-Demand pay, and a comprehensive benefits package including health, dental, vision, life insurance, 403(b), PTO, and tuition

Qualifications

• Active Colorado Nursing License.
• 1 year of home health or hospice experience preferred.
• Strong organizational and communication skills.
