Job summary

Rutherford Regional Health System is seeking a Registered Nurse (RN) - Case Management to join our team in Rutherfordton, NC. This role focuses on coordinating care across acute settings, collaborating with nurses, providers, social workers, and other MDT members to ensure safe, efficient care and positive patient outcomes.

Qualifications

• Current RN license to practice in NC.
• 2+ years of Case Management experience in an inpatient/acute care setting.
• Proficiency with multiple clinical software systems.
• Bachelor's degree in nursing preferred.

Responsibilities

• Monitors and manages patient care to promote continuity, optimal outcomes, satisfaction, cost efficiency, and regulatory compliance.
• Consults regularly with nursing staff and the multidisciplinary team to evaluate patient status and appropriateness of care, including admission, length of stay, transfers, and discharge.
• Leads and coordinates discharge planning, including transfers to other facilities, referrals to community resources, and discharge education.
• Monitors patient and family satisfaction; responds to questions and concerns from patients, families, and payors.
• Performs chart reviews to identify and address issues related to service delivery, outcomes, compliance, cost, and reimbursement.
• Communicates effectively with providers, payors, and ancillary services to remove barriers and ensure timely care progression.
• Accurately documents case management activities, plans, and outcomes.
• Participates in performance improvement and required continuing education.
• Supports department operations by working alternate shifts/units as needed; may be assigned on-call.
• Performs other duties as assigned.
