Job summary

Rutherford Regional Health System, part of Lifepoint Health, is seeking an RN Case Manager for inpatient/acute care. The role emphasizes care coordination, discharge planning, and collaboration with nursing, social work, and other clinicians to ensure safe, efficient patient progress.

The position requires an NC RN license, with an Associate degree required and BSN preferred. Schedule includes weekday hours with every fourth weekend; on-site in Rutherfordton, NC.

Qualifications

• Current NC RN license in good standing.
• 2+ years of Case Management experience in inpatient/acute care.
• Associate's Degree in Nursing is required; BSN preferred.

Responsibilities

• Monitors and manages patient care to promote continuity, outcomes, satisfaction, and cost efficiency.
• Collaborates with the multidisciplinary team to evaluate patient status and care appropriateness (admission, length of stay, transfers, discharge).
• Leads discharge planning, including transfers and referrals to community resources.
• Documents case management activities, plans, and outcomes accurately.
• Participates in performance improvement and continuing education.

Our Case Management team supports patients across a variety of acute care settings, partnering closely with nursing, providers, social work, and other multidisciplinary team members to help ensure safe, effective care coordination. How you'll contribute

• Monitors and manages patient care to promote continuity, optimal outcomes, satisfaction, cost efficiency, and regulatory compliance.
• Consults regularly with nursing staff and the multidisciplinary team to evaluate patient status and appropriateness of care, including admission, length of stay, transfers, and discharge.
• Leads and coordinates discharge planning, including transfers to other facilities, referrals to community resources, and discharge education.
• Monitors patient and family satisfaction; responds to questions and concerns from patients, families, and payors.
• Performs chart reviews to identify and address issues related to service delivery, outcomes, compliance, cost, and reimbursement.
• Communicates effectively with providers, payors, and ancillary services to remove barriers and ensure timely care progression.
• Accurately documents case management activities, plans, and outcomes.
• Participates in performance improvement and required continuing education.
• Supports department operations by working alternate shifts/units as needed; may be assigned on‑call.
• Performs other duties as assigned.

What we're looking for

Applicants should have a current RN license to practice in NC (NC licensure or multi‑state (compact) license).

Associate's Degree in Nursing required, Bachelor's Degree in Nursing preferred.

2+ years of Case Management experience in an Inpatient/Acute Care setting required.

Additional requirements include

• Strong critical thinking and decisive judgment
• Decisive judgment and the ability to work with minimal supervision.
• Must be able to work in a stressful environment and take appropriate action.
• Proficiency with multiple clinical software systems
