Job summary

Seva Hospice in Visalia, CA is seeking an experienced RN Case Manager to join our on-site hospice team. This full-time role includes an on-call weekend rotation and serves Tulare County and surrounding communities.

You will provide expert symptom management, emotional support, and compassionate care to terminally ill patients, adhering to safety standards and ethical guidelines. CA RN license and CPR certification are required.

Qualifications

• Must have at least 1 year nursing experience.
• Hospice experience preferred (1 year).
• Knowledge of nursing care processes and medication guidelines/side-effects.
• Commitment to safety precautions and high ethical standards.
• Proficient reading/writing English and strong communication skills.
• Good judgement and attention to detail with empathy and patience.
• CA Registered Nurse license required and current CPR certification.

Responsibilities

• Provide care and support to terminally ill patients.
