Job summary

Heart to Heart Hospice is seeking an RN Case Manager in Indiana to provide comprehensive assessments and coordinate high-quality hospice care. You will implement and revise patient plans, collaborate with the care team, and support patients and families with on-call responsibilities.

Qualified candidates will be RNs with a current license, a valid driver's license, reliable transportation, and up-to-date auto liability insurance.

Qualifications

• Graduate of an accredited nursing program (Diploma/Associate/Baccalaureate).
• Current State Registered Nurse License.
• Two years of RN experience preferred.

Responsibilities

• Conduct initial and ongoing comprehensive assessments of patients and ADLs, addressing physical, psychosocial, environmental needs and risk of grief.
• Implement the individualized plan of care (POC) and revise as necessary.
• Collaborate with patient, caregivers, physicians and healthcare professionals to coordinate care.

What You Must Have:

What You Will Do:

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
