Job summary

Heart to Heart Hospice is seeking an RN Case Manager in Flint, MI. The role focuses on initial and ongoing assessments of patients with terminal illness, development and revision of individualized care plans, and coordinating care among families, physicians, and the Inter-Disciplinary Group to ensure seamless patient support.

The position requires RN licensure, a valid driver's license with reliable transportation, and preferably two years of clinical RN experience.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License, reliable transportation, and provide ongoing auto liability insurance.
• Two years of RN experience; preferred in a healthcare/clinical setting.

Responsibilities

• Conduct initial and ongoing comprehensive assessments of the impact of terminal disease on patients’ physical, psychosocial, and environmental needs and ADLs.
• Implement the individualized plan of care (POC) and revise as necessary.
• Collaborate with patient, caregiver, physicians and healthcare professionals.
• Attend Inter-Disciplinary Group (IDG) meetings and coordinate patient care planning with IDG.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Ensure timely submission of all documentation and comply with laws and policies.

What You Will Do:

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
