Responsibilities include:

• Monitoring and managing patient care to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance.
• Consulting with nursing staff and multidisciplinary teams to evaluate patient status and appropriateness of medical care, including admission, length of stay, transfer, and discharge.
• Monitoring patient and family satisfaction, responding to questions and complaints regarding care.
• Participating in discharge planning, coordinating patient transfers, and community resources.
• Providing discharge education and resource referrals to patients.
• Performing chart reviews to identify issues with service delivery, outcomes, satisfaction, compliance, cost, and reimbursement.
• Performing other duties as assigned, including occasionally performing duties of CNA, MA, or LPN.
• Attending staff meetings, in-service, projects, and committees.
• Adhering to policies and procedures of Highpoint Health with Ascension Saint Thomas.
• Working scheduled shifts, including overtime and call schedules.

Qualifications:

Minimum Education: Associate's degree required.

Required Skills: Critical thinking, decisive judgment, ability to work with minimal supervision, capable of functioning in a stressful environment.

Certifications: AHA Basic Life Support (BLS).

Licenses: Registered Nurse licensed by the State of Tennessee or a Compact state with proper Tennessee licensing requests submitted.
