This full-time evening shift includes a weekend every four weeks, with a $10,000 sign-on bonus, relocation consideration, and a comprehensive benefits package.

Qualifications

• Minimum 3 years of recent case manager experience in an acute care setting.
• Associates Degree in Nursing or Diploma in Nursing is required.
• Bachelor's Degree in Nursing preferred.
• Current FL RN license required or appropriate compact licensure; if compact, active FL RN license required within 90 days of hire.
• Advanced Practice Registered Nurse license acceptable if current and compliant.
• Certification in Case Management, Nursing, or Utilization Review preferred.

Position: RN Case Manager - Tallahassee, FL

Shifts/Schedule:

Evenings (12pm-8:30pm) |Full-Time | With 1 weekend per 4-week schedule.

What You’ll Do:

The RN CM Care Coordinator will facilitate the interdisciplinary plan of care with a focus on evaluating the appropriateness of clinical care, medical necessity, admission status, level of care, and resource management. The RN CM Care Coordinator will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization. The RN CM Care Coordinator will identify potential barriers to patient throughput and quality outcomes and will facilitate appropriate discharge plans.

Qualifications Required:

• Must have a minimum of 3 years of RECENT (Within the last year) Case Manager experience in an acute care setting. *Open to 3 years of experience on the following units: Med/Surg, Tele, Neuro, ICU, PCU, or ED* Will also consider Case manager experience in home health or insurance (3 years of acute care experience total and must have at least 1 year of acute care experience within the last 5 years)
• Associates Degree in Nursing or Diploma in Nursing required
• Bachelor’s Degree in Nursing preferred
• Current FL RN license required or appropriate compact licensure. If compact license held, active FL RN licenserequiredwithin90 days of hire
• Advanced Practice Registered Nurse license is acceptable for position if current and compliant
• Certification in Case Management, Nursing, or Utilization Review, preferred

Compensation & Benefits:

• Pay rate range: Min $31.94/hr. – Max $43.92/hr. (Based on years of experience)
• Shift Diffs: Evenings $2.50/hr., Weekends $2/hr. (Which means an additional $4.50/hr. added to base pay rate)
• Sign-On Bonus: $10,000 (Paid out in 5 installments over a 2 year commitment period)
• Relocation: Case by case
