Job summary

Community LIFE is seeking an RN Case Manager to join our team in Rostraver, PA. You will conduct in-home assessments, develop care plans, and coordinate nursing and home care services as part of a multidisciplinary team in our day center and home health programs.

The role requires a nursing degree, valid driver's license, and at least 1 year of experience with frail elderly populations; patient education and collaboration are valued. Sign-on bonus included.

Qualifications

• Graduate of an accredited nursing program.
• Minimum 1 year of experience with frail elderly populations (home care, hospice, long-term care, or community health preferred).
• Experience in patient education and interdisciplinary collaboration is a plus.
• Licensed driver’s license or valid driver’s license.

Responsibilities

• Conduct in-home assessments and develop individualized care plans.

Salary Range: $72,000.00 To $85,000.00 Annually

Currently Offering a $5,000 Sign-On Bonus for this position!

Schedule: Monday – Friday, 7:30 AM – 4:00 PM plus on-call rotation
Location: Rostraver, PA
