Job summary

Community LIFE is seeking a Registered Nurse Case Manager (RNCM) for a full-time role based in Rostraver, PA. The position combines in-home patient assessments with our day center clinic, supporting frail elderly adults in a compassionate, interdisciplinary setting.

You will coordinate nursing and home care services, participate in care planning meetings, and ensure thorough documentation while traveling to patient homes as needed.

Qualifications

• Graduate of an accredited nursing program.
• Minimum 1 year of experience with frail elderly populations.
• Valid driver’s license.
• Experience in patient education and interdisciplinary collaboration is a plus.

Responsibilities

Schedule: Monday – Friday, 7:30 AM – 4:00 PM plus on-call rotation
Location: Rostraver, PA

As an RN Case Manager, you will:

Education & Experience:

• Graduate of an accredited nursing program
• Minimum 1 year of experience with frail elderly populations (home care, hospice, long-term care, or community health preferred)
• Experience in patient education and interdisciplinary collaboration is a plus

Licensure & Certifications:

• Valid driver’s license

• Strong clinical knowledge and critical thinking
• Understanding of geriatric care needs
• Excellent communication and organizational skills
• Ability to work independently and as part of a team
• Familiarity with managed care, quality assurance, and utilization review
• Basic computer proficiency (Windows, email, word processing)
