Job summary

Moments Hospice is seeking an RN Case Manager in Racine, Wisconsin, to provide compassionate care for our patients. You will assess client needs, develop care plans, and work directly with patients and families to ensure their comfort in their final days.

Qualifications

• Licensed as a Registered Nurse / RN in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and continuously refine care plans.
• Provide hands‑on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings.
