Job summary

Moments Hospice is seeking an RN Case Manager to provide compassionate in-home care, coordinating treatment plans and educating patients and families. You will work closely with the owners and clinical leadership in a growing, entrepreneurial environment that supports clinical advancement and professional development.

Qualifications

• Licensed RN in the state you work in (Wisconsin).
• Proficiency with EMR software.
• Excellent planning, organizing and communication skills.
• Ability to solve problems autonomously while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.
