Description:Seeking an experienced RN Case Manager for an acute-care hospital travel assignment.The ideal candidate is a self-directed RN Case Manager experienced in managing complex hospitalized patients.This role involves coordinating services, facilitating timely transitions of care, and documenting accurately in the electronic medical record.Candidates must possess a strong understanding of appeals and denials, benefits eligibility, care coordination, discharge planning, and CMS guidelines.The daily patient census is around 25, and the candidate will be working with adult and geriatric populations.Floating to other areas may be required, including up to 75 miles.All candidates must be willing to float and adapt to various care settings as needed.
