Job summary

Heart to Heart Hospice seeks an RN Case Manager to assess and coordinate comprehensive care for patients with life-limiting illnesses. You will work with patients and families to implement individualized plans of care, addressing physical, psychosocial, and environmental needs.

The role requires an RN license, a driving record and transportation, and usually two years of clinical RN experience. Benefits include competitive pay, insurance, PTO, and company car eligibility.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Two years of experience as an RN preferred in a healthcare/clinical setting.
• Reliable transportation and ongoing auto liability insurance.

Responsibilities

• Conduct initial and ongoing assessments of patients with life-limiting illness and determine impact on ADLs, psychosocial and environmental needs.
• Implement and revise the plan of care as necessary based on patient needs.
• Coordinate care with patient, caregivers, physicians and other health professionals.
• Attend IDG meetings and participate in patient care planning.
• Provide on-call coverage for patients and families as assigned.
• Submit documentation in a timely manner and perform additional duties as required.

What You Must Have:

What You Will Do:

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
