Cynet Health is seeking an RN Case Manager for a 13-week home health assignment in Northampton, MA. You will work under the Clinical Team Manager to deliver skilled nursing care and comprehensive case management, coordinating care plans for patients and families.

This role requires an active RN license, BLS, and prior home health experience. Work is Monday–Friday, 8:00 AM–4:30 PM, with a focus on quality direct care and patient education.
