We’re looking for a Registered Nurse (RN) Case Manager to join our home health team. If you thrive on providing high-quality care, coordinating services, and making a difference every day, we’d love to meet you.

• Competitive Pay: $38 - $45 per hour (based on experience)

Your Role

• Perform patient assessments and develop care plans
• Complete OASIS assessments at key time points
• Coordinate services and ensure seamless patient care
• Maintain accurate documentation and progress notes
• Serve as a trusted support for patients and their families
  
  

What You Need To Succeed

• Active Connecticut RN License
• CPR Certification
• Home Health Experience Preferred
• Reliable Transportation (Valid Driver’s License & Insurance)
• Strong Communication and Organizational Skills
