StartDate: 6/30/2025 Pay Rate: $2000.00 - $2700.00

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans

MINIMUM REQUIRED QUALIFICATIONS – TN/compact RN license; BLS; 2 years acute hospital case management experience; Midas & Meditech experience; Ability to adapt quickly - expectation that candidate would be productive after 5 days of orientation

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 7:00AM - 3:00PM, M-F

SYSTEMS – Midas & Meditech

START DATE – ASAP
