Job summary

Grand Strand Medical Center is seeking an RN Case Manager to coordinate patient care, facilitate discharge planning, and advocate for patients to ensure quality outcomes during hospital stays.

You will work with physicians, nurses, patients and families to navigate complex healthcare needs and promote efficient utilization of resources in an acute care setting.

Qualifications

• Active and unrestricted Registered Nurse (RN) license.
• Graduate of an accredited nursing program.
• Current BLS certification.
• Minimum of 2 years of clinical nursing experience in the acute care setting.
• Prior case management experience required.

Responsibilities

• Assess patient needs and develop individualized discharge and transition plans.
• Coordinate patient care across the continuum, including post-acute services.
• Collaborate with physicians and staff to identify discharge barriers and develop solutions.
• Perform utilization review activities and monitor resource utilization.
• Educate patients and families regarding treatment plans and discharge instructions.
• Facilitate interdisciplinary care team meetings and communicate care progression.
• Ensure documentation meets regulatory, payer, and organizational requirements.
• Advocate for patients to ensure safe, effective, and timely care delivery.

The RN Case Manager collaborates with physicians, nurses, patients, families, and interdisciplinary teams to coordinate care, facilitate discharge planning, and promote efficient utilization of healthcare resources. This role serves as a patient advocate, helping to ensure quality outcomes, seamless transitions of care, and compliance with regulatory requirements.

Key Responsibilities

• Assess patient needs and develop individualized discharge and transition plans.
• Coordinate patient care across the continuum, including post-acute services and community resources.
• Collaborate with physicians and clinical staff to identify barriers to discharge and develop solutions.
• Perform utilization review activities and monitor appropriate resource utilization.
• Educate patients and families regarding treatment plans, discharge instructions, and available support services.
• Facilitate interdisciplinary care team meetings and communicate care progression.
• Ensure documentation meets regulatory, payer, and organizational requirements.
• Advocate for patients to ensure safe, effective, and timely care delivery.
• Maintain knowledge of healthcare regulations, reimbursement guidelines, and case management best practices.
