VNA Health is seeking a compassionate Registered Nurse Case Manager to provide in-home nursing care across the Santa Barbara area. This role offers autonomy within an interdisciplinary team and opportunities for clinical decision-making in a home health setting.

The position is full-time (40 hours/week) with rotating weekends, requires CA RN license, CPR, and a valid driver's license. Competitive hourly pay and comprehensive benefits are provided.

Qualifications

• Current and active California RN license.
• Bachelor’s degree, preferred.
• Minimum one year acute care experience required.
• One year recent home health experience preferred.
• Current CPR certification.
• Licensed driver with insured automobile.
• Management experience not required; supervising home health aides/LVNs.
• Excellent observation, verbal and written communication skills, problem solving skills; nursing skills per competency checklist.

Responsabilités

• Continuously review all aspects of each patient on caseload: utilization of services, skilled need, homebound status, documentation review, interdisciplinary communication, discharge planning.
• Conduct initial patient and family assessment to determine home care needs; provide complete physical assessment and history; admit to appropriate service level.
• Use health data to determine nursing diagnoses.
• Initiate and revise the plan of care as patient status changes.
• Develop care plans with goals based on nursing diagnosis; involve patient and family.
• Administer medications and treatments as prescribed by physician.
• Regularly re-evaluate patient nursing needs.
• Counsel patient and family on nursing and related needs.
• Ensure compliance with state, federal, local and accreditation regulations for home health services.

About the Position:

This position serves patients throughout the Santa Barbara area as part of VNA Health's Home Health team. We are seeking a compassionate and dedicated Registered Nurse Case Manager to provide personalized, high-quality nursing care to patients in the comfort of their homes. Pay:$58–$62 per hour

Schedule: Full-Time, 40 hours per week, with monthly rotating weekend shifts. Scheduling may include weekdays, weekends, and holidays based on organizational needs.

Responsibilities:

• Responsible for continuous review of all aspects of every patient on his/her caseload to include: appropriate utilization of services; ensuring continued skilled need; monitoring of homebound status; review of documentation in the medical record; maintenance of interdisciplinary communication and discharge planning.
• Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es). Admits to appropriate level of service/ care
• Uses health assessment data to determine nursing diagnosis.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
• Regularly re-evaluates patient nursing needs.
• Counsels the patient and family in meeting nursing and related needs.
• Knowledge of state, federal, local and accreditation regulations pertaining to the delivery of home health services.
