Hours

Monday‑Friday 8:30AM‑5:00PM

Responsibilities

1. Completing an initial assessment of patient and family to determine hospice care needs
2. Regularly re‑evaluating the patient’s nursing needs
3. Use assessment data to determine care plan needs
4. Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide care that is continuous and well‑coordinated
5. Counsel the patient and family in meeting end‑of‑life needs
6. Initiate appropriate preventative and palliative nursing procedures
7. Communicate with the physician regarding patient’s needs and reports any changes in the patient’s condition
8. Supervise the Hospice Aides

Education

Associate’s Degree in Nursing

Professional Experience

• Must have a State Certified RN license
• 2 years’ experience as a professional nurse, hospice experience preferred
• Must have a valid Driver’s license and reliable transportation
• Must be willing to travel
• Strong verbal and written communication skills
