Job summary

Ascend Hospice in Liberty, MO is hiring an RN Case Manager to support on-call coverage, visits, and care coordination. You will assess patients, plan and re-evaluate nursing needs, and collaborate with the care team to ensure continuous, high-quality end-of-life care.

The role requires an Associate's Degree in Nursing, a state RN license, 2 years nursing experience (hospice preferred), and willingness to travel.

Qualifications

• Must have a State Certified RN license.
• 2 years' nursing experience; hospice preferred.
• Valid driver's license and reliable transportation required.
• Willingness to travel.
• Strong verbal and written communication skills.

Responsibilities

• Complete initial assessment of patient and family to determine hospice care needs.
• Regularly re-evaluate patient nursing needs and adjust care plans.
• Use assessment data to plan care and coordinate with the healthcare team.
• Counsel patient and family on end-of-life needs.
• Communicate with physicians about patient condition and changes.
• Supervise Hospice Aides and ensure continuous, well-coordinated care.

Now Hiring – RN Case Manager – Liberty, MO

Hours:M-F 8:30am-5pm

The RN Case Manager will be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:

• Completing an initial assessment of patient and family to determine hospice care needs
• Regularly re-evaluates the patients nursing needs
• Use assessment data to determine care plan needs
• Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated
• Counsels the patient and family in meeting end of life needs
• Initiates appropriate preventative and palliative nursing procedures
• Communicates with the physician regarding patient's needs and reports any changes in the patient's condition
• Supervises the Hospice Aides

Education:

Associate's Degree in Nursing

Professional Experience:

• Must have a State Certified RN license
• 2 years' experience as a professional nurse, hospice experience preferred
• Must have a valid Driver's license and reliable transportation
• Must be willing to travel
• Strong verbal and written communication skills
