Job summary

Moments Hospice is seeking an experienced RN Case Manager to lead patient care in hospice settings. You will assess needs, coordinate care with families and physicians, and manage symptoms and comfort at end of life.

This full-time role features a Monday–Friday schedule, a generous pay range of $75,000 to $90,000 per year, and a fleet car program. Strong EMR skills and a compassionate, independent approach are essential.

Qualifications

• Active RN license in the state of employment.
• Proficiency with electronic medical records (EMR).
• Strong organizational, communication, and critical-thinking skills.
• Ability to work independently while maintaining high standards of care.
• Compassionate, patient- and family-centered approach.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plan of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and end-of-life expectations.
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
