Moments Hospice is seeking an RN Case Manager to serve as the clinical lead for a caseload in Minnesota. You will manage symptoms, coordinate care across disciplines, and support families through end-of-life care.

This full-time, field-based position offers a competitive salary and a fleet car program. You will have direct access to leadership and meaningful opportunities to influence patient outcomes and care standards.

• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life.
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.

RN Case Manager – Hospice

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM

Qualifications:

• Active Registered Nurse (RN) license in the state you work in
• Proficiency with electronic medical records (EMR)
• Strong organizational, communication, and critical-thinking skills
• Ability to work independently while maintaining high standards of care
• A compassionate, patient- and family-centered approach
• Valid driver's license and reliable vehicle with current insurance
