Job summary

Moments Hospice is seeking an experienced RN Case Manager to serve as the trusted clinical lead for patients and families. You will manage symptoms, coordinate care, and provide guidance during the final chapters of life, delivering comfort and dignity in a compassionate setting.

This full-time, field-based role offers a Monday–Friday schedule (8:30 AM–5:00 PM), a competitive salary of $75,000–$90,000, and benefits including medical, dental, vision, life, disability, PTO, a fleet car program,

Qualifications

• Active RN license in the state you work in.
• Proficiency with EMR systems.
• Strong organizational and communication skills.
• Ability to work independently with high standards of care.
• Compassionate, patient- and family-centered approach.
• Valid driver\'s license and reliable vehicle with current insurance.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs
• Develop, implement, and update individualized plan of care in partnership with patients and families
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
