Job summary

Community LIFE is seeking a Registered Nurse Case Manager to conduct in-home assessments, develop individualized care plans, and coordinate nursing and home care services for frail elderly patients. The role involves collaboration with a multidisciplinary team in our day center clinic and travel to patient homes within the service area.

RN Case Manager

Full Time 40 Professional Homestead, PA, US

Salary Range: $72,000.00 To $85,000.00 Annually

Currently offering a $5000 sign on bonus for this position!

Schedule: Monday – Friday, 7:30 AM – 4:00 PM
Location:Homestead, PA
On-call: Occasional rotation required

As an RN Case Manager, you will:

• Conduct in-home assessments and develop individualized care plans
• Coordinate nursing and home care services
• Collaborate with a multidisciplinary team in our day center clinic

Education & Experience:

• Graduate of an accredited nursing program

Licensure & Certifications:

• Valid driver’s license

Skills:

• Strong clinical knowledge and critical thinking

• Ability to work independently and as part of a team
• Familiarity with managed care, quality assurance, and utilization review
• Basic computer proficiency (Windows, email, word processing)
