Job summary

Community LIFE Homestead, PA is seeking an RN Case Manager to deliver holistic care to frail elderly clients in home and clinic settings. You will perform in-home assessments, develop individualized care plans, and coordinate nursing and home care services.

Join a multidisciplinary team, participate in care planning meetings, document thoroughly, and travel locally to patients. A valid nursing license and at least one year of relevant experience are required; a driver’s license is needed.

Qualifications

• Graduate of an accredited nursing program.
• Minimum 1 year of experience with frail elderly populations (home care, hospice, long-term care, or community health preferred).
• Experience in patient education and interdisciplinary collaboration is a plus.

Schedule: Monday – Friday, 7:30 AM – 4:00 PM
Location:Homestead, PA
On-call: Occasional rotation required

As an RN Case Manager, you will:

Education & Experience:
