Job summary

Serenity Hospice is seeking an RN Case Manager to deliver exceptional hospice care, comfort and support to patients. You will develop individual care plans, perform regular check-ups, and communicate with family members as needed.

This role covers Mercer County with travel as required. The candidate must hold a current NJ RN license, have at least one year of hospice and hospital setting experience, and demonstrate strong communication and interpersonal skills.

Qualifications

• Current NJ RN License in good standing.
• One year Hospice experience required.
• One year hospital setting experience required.
• Willing to travel to assigned areas.
• Reliable transportation with liability insurance.
• Strong communication and interpersonal skills.

Responsibilities

• Complete initial assessment of patient and family to determine hospice care needs.
• Regularly reevaluate the patient’s nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the healthcare team, patient and family to deliver continuous, well-coordinated care.
• Counsel patient and family in meeting end-of-life needs.
• Initiate appropriate preventative nursing procedures.
• Communicate with physician regarding patient needs and changes in condition.
• Supervise Hospice Aides and LPNs.

Serenity Hospice is looking to add a dedicated and compassionate RN Case Manager to delivering exceptional care, comfort and support to patients. They will develop plans for individual needs, perform regular check-ups and communicate with family members as needed. This position will oversee Mercer County.

Responsibilities:

• Complete an initial assessment of patient and family to determine hospice care needs

• Regularly re-evaluates the patients nursing needs

• Use assessment data to determine care plan needs

• Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide care that is continuous and well-coordinated

• RN counsels the patient and family in meeting end of life needs

• Registered Nurse initiates appropriate preventative nursing procedures

• Communicates with the physician regarding patient's needs and reports any changes in the patient's condition

• Supervises the Hospice Aides and LPN's

Requirements:

• Current NJ RN License in good standing

• One year Hospice Experience REQUIRED

• One year of Hospital Setting Experience REQUIRED

• Willing to travel to the assigned areas

• Must have reliable transportation with liability insurance

• Strong communication and interpersonal skills.
