Job summary

WissenCare is seeking a dedicated RN Case Manager to coordinate inpatient care and collaborate with multidisciplinary teams to optimize patient outcomes across the continuum of care.

The role emphasizes effective discharge planning, resource utilization, and helping clients navigate complex healthcare pathways within the affiliated facilities. Ideal candidates hold a BSN, have 2+ years acute care and case management experience, and are proficient with Cerner or Epic systems.

Qualifications

• BSN required.
• Minimum 2 years acute care nursing experience.
• Minimum 2 years of Case Management experience.
• Cerner & Epic experience preferred.
• CCM or ACM certification preferred.
• ED experience is a strong plus.
