Heart to Heart Hospice is seeking an experienced RN Case Manager to assess and coordinate comprehensive home-based patient care in the Dallas area. The role requires an active RN license, reliable transportation, and 2+ years of clinical nursing experience.

You will develop and implement care plans, collaborate with IDG members, and ensure timely documentation and regulatory compliance. This full-time position offers competitive pay and benefits in a supportive team dedicated to patient-centered

Competenze

• Graduate of an accredited Diploma, ADN, or BSN program.
• Current State RN license.
• Driver's license with reliable transportation and valid auto liability insurance.
• Two years of RN experience preferred in healthcare/clinical setting.

Mansioni

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on patient needs and ADLs.
• Implement the individualized POC and revise as necessary.
• Collaborates care among patient, caregiver, physicians and healthcare professionals.
• Attend IDG meetings and coordinate patient care planning with the IDG.
• Perform on-call responsibilities and provide on-call service to patients/families.
• Timely submission of all documentation
• Performance of other duties as required

What You Must Have:

• Current State Registered Nurse License
• Current Driver's License, reliable transportation,and provide ongoing valid and current auto liability insurance
• Two years of experience as an RNpreferred in a healthcare/clinical setting

What You Will Do:

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
