Shine Residential seeks an experienced RN Case Manager to coordinate medical care for individuals with developmental disabilities in home and community settings. You will assess health needs, manage medications, and work with families, DSPs, and regional centers to implement individualized plans.
You will travel to client homes, conduct assessments, provide hands-on care, and ensure compliant, patient-centered care that improves quality of life.
We are seeking an RN Case Manager to focus on long-term wellness, independence, quality of life, stability, and daily health maintenance. In this role, you will bridge clinical health care needs with community-based, person-centered independent living.
Health and Medical Assessments: Evaluate the physical and functional health status of current and new clients with developmental or intellectual disabilities living in their own apartments or shared homes. Identify medical, behavioral, and social determinants of health that may create barriers to the client's treatment plan.
Hands-On Client Care: Provide hands-on nursing care in clients' homes. Travel to client homes, community settings, and physician offices to conduct visits and deliver care coordination services. Monitor client progress and needs (wellness, medical, and specialty treatment) and report changes in condition to the client's physician and health care team.
Individual Program Plan (IPP) & Care Coordination: Partner with the client, their family, regional center service coordinators, direct support professionals (DSPs), and physicians to build and update health-related goals.
Client & Family Education and Support: Educate clients about their care plan and health care options, and support them in making informed decisions about their own care. Provide education on disease management, medications and self-administration, and safe home and community practices. Support adherence to the treatment plan, recommend community resources as needed, and offer emotional support to clients and their families.
Medication Oversight and Management: Oversee medication regimens; train direct support staff and caregivers on safe medication assistance, accurate documentation, and side-effect monitoring in accordance with applicable California regulations; and coordinate with prescribers and pharmacies.
Staffing and Supervision: Provide clinical guidance, health-safety training, and protocol instruction to direct support professionals (DSPs) and program managers who work hands-on in the field.
Crisis and Emergency Response: Act as an escalation point or clinical resource for medical emergencies, sudden changes in a client's health, or hospital-to-home transitions.
Advocacy and Liaison Work: Advocate for the client's medical and personal rights, ensuring they receive proper accommodations from outside medical specialists, therapists, and community programs.
Licensure: Active, unrestricted Registered Nurse (RN) license issued by the California Board of Registered Nursing, maintained in good standing throughout employment.
Education: Graduate of an accredited nursing program (Associate Degree in Nursing or Bachelor of Science in Nursing).
Experience: Minimum of two (2) years of RN experience, including at least one (1) year of case management, community health, home health, or work with individuals with intellectual and/or developmental disabilities.
Regulatory Knowledge: Working knowledge of the Lanterman Developmental Disabilities Services Act, California Code of Regulations Titles 17 and 22, the regional center system and IPP process, and HIPAA confidentiality requirements.
Clinical Skills: Demonstrated ability to conduct health and functional assessments, develop health care plans, manage complex medication regimens, and recognize changes in condition that require escalation.
Training Ability: Ability to teach and coach direct support professionals with varying levels of experience on health, safety, and medication procedures.
Documentation: Strong, accurate, and timely clinical documentation skills, with proficiency in electronic records, email, and Microsoft Office or Google Workspace.
Incident Reporting: Understanding of mandated reporter obligations and special incident reporting requirements, with the ability to report promptly and accurately.
Communication: Excellent verbal and written communication skills and the ability to work collaboratively with clients, families, regional center staff, physicians, and community partners.
Certifications: Current CPR and First Aid certification (BLS preferred), or the ability to obtain it before the start date.
Transportation: Valid California driver's license, reliable transportation, and proof of current auto insurance, as this position requires regular travel to client homes and community settings.
Clearances: Ability to pass a criminal background clearance (DOJ/FBI Live Scan), health screening, and TB test as required by state regulations.
Ability to drive and travel throughout the service area, walk and climb stairs during home visits, and lift or assist with up to 25 pounds. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of this position.
