Job summary

Sweetwater Care in Los Angeles, CA is seeking a Registered Nurse (RN) to provide skilled nursing care in our Intermediate Care Facility for individuals with disabilities. You will implement health care plans, administer medications, and collaborate with a multidisciplinary team to support residents.

This full-time role offers a 9:00 AM–5:00 PM schedule, Monday to Friday, with on-call evenings, weekends, and holidays as needed.

Qualifications

• Valid Registered Nurse (RN) license in the state of California.
• 1 year experience as an RN; required.
• CPR certification.
• Strong communication and interpersonal skills.
• Ability to work effectively in a team-oriented environment.
• A genuine passion for providing quality care to individuals with disabilities.

Responsibilities

• Provide skilled nursing care to individuals with disabilities in an Intermediate Care Facility setting.

Job description

Schedule: 9:00 AM to 5:00 PM, Monday - Friday; with on-call evenings, weekends and holidays, as needed.

Facility Information:

• Our facilities are Intermediate Care Facilities for individuals with disabilities (ICF/DD-H and ICF/DD-N)
• Caregiver (DSP) to people supported ratio is usually 1:3, sometimes as high as 1:1 support
• Nursing caseload typically ranges from 6-15 residents
• Our home sizes are small, including 6-bed and 15-bed facilities
• High-acuity care is limited or prohibited by facility regulations
• Our Immediate Care Facilities provide wound care per licensing requirements, ranging from simple wounds to a maximum of stage I pressure ulcers
• Hospice care is very limited and is provided collaboratively with professional hospice care providers who come to our homes

Registered Nurse (RN) Essential Responsibilities:

Registered Nurse (RN) Qualifications:

• Valid Registered Nurse (RN) license in the state of California
• One (1) year experience as an RN; required
• CPR certification
• Strong communication and interpersonal skills
• A genuine passion for providing quality care and support to individuals with disabilities
• Ability to work effectively in a team-oriented environment
• Willingness to adapt to the needs of the residents and facility
