Role Description

This is a full-time field role for an RN Case Manager, located in Orange County, CA,. The RN Case Manager will be responsible for managing the care of hospice patients, coordinating with interdisciplinary teams, developing and implementing care plans, and providing support and education to patients and families. Daily tasks include assessing patient needs, administering treatments, monitoring progress, and ensuring compliance with healthcare regulations and standards.

Qualifications

• Registered Nurse (RN) license in the state of California
• Experience in hospice or palliative care
