Job summary

Valor Healthcare is seeking a qualified Registered Nurse to join our team supporting the Vermont Army National Guard’s non-clinical case management program in Colchester, VT. This role focuses on coordinating care, ensuring timely access to services, and maintaining HIPAA-compliant documentation within a federal workflows.

The contract position requires U.S. citizenship, an active RN license (Vermont or ability to obtain), and a minimum of two years in case management or utilization management.

Qualifications

• Active, unrestricted RN license (VT or ability to obtain).
• BSN preferred.
• Minimum 2 years of case management, care coordination, or utilization management.
• Strong organizational and documentation skills.
• US Citizenship required.
• Ability to pass federal background screening.

Responsibilities

• Support Periodic Health Assessments and Soldier Readiness Processing by providing case management and coordinating care activities.
• Establish and maintain electronic case management profiles within required timelines following provider referral.
• Prepare and contribute to monthly case status reports by tracking Soldier care progression and readiness outcomes.
• Apply Individual Medical Readiness and deployment regulations while obtaining required medical release documentation.
• Maintain HIPAA compliance during all case management activities.
• Document medical and administrative information across required systems, ensuring accuracy and timeliness.
• Prepare and review case files for Medical Evaluation Board and Physical Evaluation Board processes.
• Communicate with Government personnel to track compliance and address case management requirements.
• Engage with Soldiers to explain requirements, obtain documentation, and ensure understanding of responsibilities and privacy rights.
• Manage medical profiles and contribute to reporting on profile status and updates.
• Identify and coordinate submission of medical documents in accordance with required processing timelines.
• Participate in monthly case reviews, presenting case information and supporting follow-up actions.

Position SummaryThe Vermont Army National Guard (VTARNG) Office of the Deputy State Surgeon has a requirement to provide Non Clinical Case Management Services for VTARNG service members (SM) thereby promoting fitness and personal wellness for operational readiness and deployability. Non-Clinical case Managers coordinate all aspects of the care of individual patients. They ensure proper utilization of services and resources and provide assistance within, between, and outside of VTARNG facilitiesSchedule: The contractor shall provide services between the hours of 0730-1630 on Monday through Friday. The contractor is required to support weekend soldier readiness events. This contract honors 11 paid federal holidays.Location: Performance shall be at VTARNG State Medical Detachment, Green Mountain Armory, Camp Johnson, Colchester, VT 05446.RequirementsCore Responsibilities

Qualifications

• Bachelor of Science in Nursing (BSN) preferred
• Minimum 2 years of case management, care coordination, or utilization management experience
• Strong organizational and documentation skills
• Ability to work independently in a structured federal environment
• U.S. Citizenship required
• Ability to pass federal background screening

• Experience supporting federal or military-affiliated programs
• Experience with medical readiness, occupational health, or wellness programs
