Tennessee Quality Care Hospice in Clarksville, TN is seeking an RN Case Manager to join our growing team. This field-based role manages a patient caseload, coordinates care, and supports patients and families in the home setting.

Responsibilities include assessment, care planning, interdisciplinary communication, regular home visits, and ensuring timely documentation in patient records. Join a compassionate, patient-centered hospice team dedicated to high-quality care.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Current CPR and First Aid certifications.
• Knowledgeable of quality assessment and assurance procedures.
• Valid driver's license and proof of insurance.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation of nursing care.
• Obtain data from patient physical, psychological, social, and spiritual factors affecting needs and coordinate with interdisciplinary team.
• Initiate communication with attending physician and other agencies to coordinate optimal care.
• Provide regular visits to reassess patient needs and update the care plan.
• Perform clinical pronouncement of death and provide family support.
• Maintain accurate patient records, charts, and care plans in a timely manner.
• Supervise and document delivery of home care by Home Health Aide per guidelines.

Tennessee Quality Care Hospice is seeking an RN Case Manager to join their growing team in Clarksville. This field-based position is responsible for managing a caseload of patients, providing therapeutic nursing, coordinating care, and supporting patients and their families in the comfort of their homes.

Territory:

Clarksville, TN and surrounding counties

Schedule:

Monday thru Friday 8am-4:30pm. Estimated on-call expectation- 5 days a year.

What You’ll Do:

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
