Job Description

• This person will be working directly with members to assess, plan, and coordinate care.
• They will have a caseload of about 75 members.
• Perform care management duties to assess, plan and coordinate all aspects of medical and supporting services across the continuum of care for select members to promote quality, cost effective care.
• Monday‑Friday 8 a.m.–5 p.m., Full time with medical benefits offered.
• This is an immediate Right to Hire opening.
• Pay range $32/hr–$35/hr salary negotiated based on relevant experience.
