Job summary

Interim HealthCare Inc. in Ohio is seeking a Registered Nurse Case Manager to provide high-quality home health care in the Saint Clairsville region. You will conduct in-person assessments and oversee patient care plans with a team of aides, LPNs and RNs.

The role offers flexible scheduling, competitive pay, daily pay option, and no overtime requirements, with a focus on 1:1 patient care and reducing readmissions through preventative care.

Qualifications

• Current, unrestricted RN license in the state where the position is located.
• Current CPR/AED/BLS/First Aid certification.
• Reliable transportation to/from care sites and/or work locations.
• One year of professional RN experience in home health or similar setting; case management experience preferred.
• OASIS experience preferred.
• Trach/ventilator experience preferred, not required.

Responsibilities

• Conduct in-person patient interviews and comprehensive physical assessments.
• Oversee the implementation and ongoing assessment of the patient's plan of care through management of home health aides, LPNs, RNs, and other caregivers.
• Communicate patient conditions and collaborate with appropriate providers to deliver care as needs evolve.
• Educate patients and families on proper home health care procedures (wound care, IV administration, medication management).
• Work to decrease readmissions by promoting preventative care and ensuring continuity of care.

• One (1) year of professional experience practicing as a Registered Nurse (RN) in home health or similar setting; previous case management/utilization review experience preferred.
• OASIS experience preferred.
• Practical trach and/or ventilator experience preferred, not required.
