Job summary

Interim HealthCare Inc. is seeking a Registered Nurse Case Manager to cover the Saint Clairsville, OH area. You will conduct in-person assessments, supervise care plans, and coordinate with a team of aides and clinicians to ensure patient needs are met.

The role emphasizes education for patients and families, preventive care to reduce readmissions, and collaboration with providers. A valid RN license and CPR/BLS certification are required, with a strong focus on 1:1 patient care.

Qualifications

• Active RN license in the state where the position is located.
• Current CPR/AED/BLS/First Aid certification required.
• Reliable transportation to care sites and/or work locations.
• At least 1 year of RN experience in home health or similar settings; case management preferred.
• OASIS experience is a plus; ventilator/trach experience preferred but not required.

Responsibilities

• Conduct in-person patient interviews and comprehensive physical assessments.
• Oversee care plan implementation with home health aides and clinicians.
• Communicate patient conditions to providers to adjust care as needs evolve.
• Educate patients and families on home health procedures such as wound care and IV administration.
• Help decrease readmissions by promoting preventive care and continuity of care.

As a Registered Nurse Case Manager (RNCM), you will:

• Conduct In Person patient interviews and comprehensive physical assessments.
• Oversee the implementation and ongoing assessment of the patient's plan of care through the management of home health aides, LPNs, RNs, and other caregivers.
• Communicate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve.
• Provide education to patients and families on proper home health care procedures. Ie. Wound care, IV administration, medication management.
• Work to decrease readmissions by promoting preventative care and ensuring continuity of care.

To qualify as a Registered Nurse Case Manager (RNCM) with us, you will need:

• Licensure: Current unrestricted license to practice as a Registered Nurse (RN) in the state associated with this position
• Current CPR/AED/BLS/First Aid certification
• Reliable transportation to/from care sites and/or work locations.
• One (1) year of professional experience practicing as a Registered Nurse (RN) in home health or similar setting; previous case management/utilization review experience preferred.
• OASIS experience preferred.
• Practical trach and/or ventilator experience preferred, not required.
