Job summary

Interim HealthCare seeks an RN Case Manager to coordinate and deliver skilled nursing care to patients in their homes. You’ll complete comprehensive assessments, develop individualized care plans, and oversee implementation of services that promote safety, independence, and quality of life.

You’ll collaborate closely with physicians, therapists, aides, patients, and families to ensure continuity of care in a nationwide home health and hospice network.

Qualifications

• Active RN license in Indiana.
• Experience in home health nursing is desirable.
• Strong assessment and care planning skills.

Responsibilities
