Job summary

Crescent Hospice in Augusta is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to join our dedicated nursing team providing compassionate end‑of‑life care throughout the community.

As an RN Case Manager, you will deliver hands‑on nursing care, coordinate services, educate patients and families, and support the interdisciplinary team to ensure comfort, dignity, and high‑quality care. Monday to Friday schedule with on‑going opportunities for growth.

Qualifications

• Hold current unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
• Proficient with computers and EMR systems.

Responsibilities

• Respond to clinical referral information with care and expertise, addressing inquiries with compassion.
• Collaborate with Clinical Director to assess eligibility and suitability for hospice services.
• As an RN Case Manager, deliver direct care and coordinate services to ensure patient comfort and quality of life.
• Nursing with Heart: apply nursing skills to ease pain and bring solace to patients under care.
• Educate and advocate for patients and families to create personalized care plans.
• Participate in on-call duties as part of the team.

Are you a compassionate Registered Nurse (RN) who finds purpose in supporting patients and families through meaningful moments of care? Crescent Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Schedule: Monday - Friday, 8 am - 4:30 pm

Territory: Augusta

In this rewarding role, you’ll deliver hands‑on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. Responsibilities

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

$78,000 - $83,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
