Job summary

Crescent Hospice is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to join our compassionate nursing team delivering hands-on hospice care throughout the Tri-County area of Berkeley, Charleston, and Dorchester. Schedule is Monday–Friday, 8 am–4:30 pm, with a $7,500 sign-on bonus.

You will coordinate services, guide patients and families through the hospice journey, and work within an interdisciplinary team to provide The Best Care Possible—grounded in compassion and clinical

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

• Educator and Advocate: Educate patients and families, encouraging active participation in creating personalized care plans.
• Call of Compassion: Share responsibilities of call duties to be there when it matters most.

Are you a compassionate Registered Nurse (RN) who finds purpose in supporting patients and families through meaningful moments of care? Crescent Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Schedule: Monday - Friday, 8 am - 4:30 pm

$78,000 - $83,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
