What You'll Do

• Perform program-specific patient outreach, education, and recruitment.
• Consult with patients’ physicians and/or Medical Directors about high-priority patients and potential care management plans.
• Initiate contact with eligible patients to perform initial assessments and develop care management plans.
• Implement high-quality, cost-effective care plans using case management principles.
• Communicate effectively with patients, families, and physicians to build trust and engage patients in health promotion activities.
• Provide direct care coordination and encourage enrollment in disease and case management programs.
• Determine appropriateness of hospital admissions and disease management programs.
• Communicate with healthcare clinicians about patient care and follow-up plans.
• Establish a communication schedule for periodic contact with providers and patients.
• Attend and present case reviews at practice and care coordination meetings.

Requirements

• Education:
  • Bachelor's Degree in Nursing required, Master's Degree in Nursing preferred.
• Experience:
  • Minimum 1–3 years of oncology care experience within the past five years required.
  • Minimum 1 year of case management experience in an outpatient setting within the past five years required.
  • Experience with complex patient populations (e.g., solid tumors, hematologic malignancies) strongly preferred.
  • Health care reimbursement experience strongly preferred.Knowledge of insurance authorization processes and healthcare systems strongly preferred.
• Licenses & Certifications:
  • Current licensure as a Registered Nurse in the Commonwealth of Massachusetts required.
