Option Care Health in Chantilly, VA is seeking an RN or Pharmacist with at least 2 years in healthcare to grow referral relationships, educate providers, and support patient transitions to home or alternate site care.

You will collaborate with Account Managers and Regional Sales Directors to execute growth plans, review medical records, coordinate admissions, and ensure timely, quality discharge and therapy independence.

Qualifications

• Licensed RN or Pharmacist with state license.
• At least 2 years of healthcare experience.
• Strong relationship-building across organizational levels.
• Familiar with ACHC and URAC standards.
• Customer service oriented

Responsibilities

• Prospect and grow referrals and educate on options.
• Coordinate discharge and home care transitions.
• Collaborate with Account Manager and Regional Sales Director.
• Review medical records and obtain authorization information.
• Educate patients and families to support timely discharge.
