Job summary

Qualifications

• RN license current in Illinois.
• Graduate of an accredited nursing program.
• CPR/AED certification required, or obtained within six months.
• Long-term care experience preferred.
• Critical thinking and strong communication skills.
• Ability to stand, walk, and lift 200+ lbs with assistance.
• Proficiency with electronic documentation.

Responsibilities

• Assess residents' health status on admission and ongoing.
• Develop and revise care plans with the IDT team's input.
• Document care plans, interventions, and orders accurately.
• Use SBARR for timely physician notification of changes.
• Assist with ADLs, respond to call lights, and ensure safety.
• Participate in QAPI processes for quality improvement.
