Job summary

IHA is seeking a dedicated Registered Nurse to provide professional nursing care in an outpatient setting. The RN will triage patients by phone and during visits, ensuring timely access to the appropriate level of care and reducing care fragmentation.

Collaborating with physicians, care managers, and the broader care team, the RN will support transitions of care, document findings, and perform medication reconciliation. HIPAA compliance and patient safety remain central to daily duties.

Qualifications

• Graduate of an accredited RN program.
• Valid, unrestricted RN license in Michigan with CPR certification.
• 1–2 years of clinical experience preferred; previous nursing experience not required.

Responsibilities

• Triage patient needs by phone and in visits to ensure timely access to care.
• Document findings clearly and timely in the medical record.
• Collaborate with providers and care team to coordinate patient care.
• Enter orders per provider direction and follow protocols.
• Perform nursing education and support patient engagement.
• Assist with transitions of care and post-discharge calls as needed.
• Maintain patient confidentiality and HIPAA compliance.
• Participate in office/process improvement initiatives.
