Qualifications

• RN licensure valid in the state of the community location.
• Minimum of five years of clinical experience in case management, discharge planning, or a similar referral navigation role.
• Strong background in healthcare marketing and a proven track record of driving SNF admissions and managing payor mix.
• Ability to work independently, manage multiple priorities, and meet deadlines.
• Willingness to travel locally for community outreach and hospital visits.

Responsibilities

• Referral Management & Lead Generation: Serve as the primary clinical point of contact for external referrals, evaluating placement appropriateness and developing a network of referral sources.
• SNF Census Growth & Payor Mix Optimization: Directly oversee census growth and optimize payor mix for financial health.
• Clinical Liaison & Placement Optimization: Use RN expertise to assess resident needs and facilitate appropriate placement.
• Marketing & Community Outreach: Plan and execute referral marketing strategies and represent the community externally.
• Administrative & Reporting: Maintain inquiry logs and referral tracking; provide regular reports to leadership.
• Quality & Compliance: Ensure adherence to policies, procedures, and regulatory requirements.
