Barnes-Jewish Hospital at Washington University Medical Center in St. Louis is seeking a registered nurse to coordinate care for designated patient populations across the continuum.

The role focuses on patient assessment, education, and collaboration with the health care team to ensure safe, evidence-based care. Minimum qualifications include an RN license and a nursing degree; 2-5 years of experience is preferred.

Qualifications

• RN license is required.
• Nursing Diploma or Associate's degree.
• Bachelor's Degree in Nursing preferred.
• 2-5 years of nursing experience preferred.
• No supervisor experience required.

• Facilitates patient care by assessing and evaluating patient's physical, psychosocial and emotional needs.
• Coordinates the delivery of safe and appropriate patient care governed by evidence-based clinical practice.
• Monitors care and data to optimize patient outcomes.
• Facilitates the education of patients and families. Acts as a resource to the health care team.
• Ensures actions meet and support the overall goals of the business.

• Full Time position Monday - Friday
  • (5) 8hr shifts some evenings may be required
• No Holidays, No Weekends
• Neuro backgroud preferred or ICU or ER experience
• Min 3 years RN experience preferred

Role Purpose

Coordinates the health care of select patient populations/designated service across the continuum of care. Responsible for coordination of patient contact, patient education, prepares patient/family for procedure/test in collaboration with the healthcare team.
