Job summary

Bishop Gadsden Episcopal Retirement Community in Charleston, SC is seeking a compassionate Registered Nurse Care Navigator to help older adults and their families navigate healthcare decisions, coordinate services, and maintain the highest quality of life. This is a non-bedside role that leverages your nursing degree in a patient-centered, relationship-building environment.

You will coordinate appointments, develop care plans, monitor needs, and partner with families and interdisciplinary teams

Qualifications

• Active RN license in SC.
• Minimum 2+ years of nursing experience.
• Knowledge of senior services and aging resources.
• Ability to accurately and efficiently document care in a timely manner.
• Strong communication, organization, and customer service skills.
• Valid SC driver's license with clean driving record over the past 3 years.

Responsibilities

• Guide residents and families through healthcare and aging-related decisions.
• Coordinate appointments, services, and community resources.
• Develop and support individualized care plans.
• Monitor resident needs and communicate changes in condition or level of care.
• Complete assessments and Long Term Care insurance documentation.
• Partner with families and interdisciplinary teams to ensure excellent resident support.
• Accompany residents to medical appointments as needed.
