Responsibilities

• Coordinate an assigned group of patients through the health care system for inpatient care and safe transition to outpatient care.
• Collaborate with physicians, nurses and other health care workers to provide disease management assessment, treatment and follow-up evaluation.
• Monitor patient care processes and collect data to evaluate timeliness and appropriateness of care.
• Maintain awareness of state and national health care trends and respond with initiatives to enhance provision of care and resource utilization.
